    • By continuing, you acknowledge that: you are 18 years of age or older; you agree to register for a membership into TruMedical Accident Plus Plan; and acknowledge that you have read, understand and agree to the TruMedical Accident Plus Plan Terms and Conditions and hereby authorize the plan administrator, NBFSA, for TruMedical Accident Plus Plan to deduct $--.-- from your account on a monthly basis. You understand that this authority shall remain in force until you notify TruMedical Accident Plus Plan or their plan administrator NBFSA of its cancellation; You have given accurate information provided during this enrollment process; and this is not insurance. If you are not satisfied within 30 days after the effective date, you can call 888-424-4186. The Program will then be canceled and any Charges /Fees paid, as shown above, will be refunded.

    All members of the TruMedical Accident Plus 10000, 5000, and 2500 Plans are enrolled into American Advantage Association to be eligible to receive these benefits.

    Group Accident Insurance is issued on Form Series MP-1300 by Guarantee Trust Life Insurance Company, Glenview, IL. This product and its features are subject to state availability and may vary by state. Certain exclusions and limitations may apply. The Insurance rates associated for the Accident plans are as follows: Individual 2,500 Plan: $4.92 / Family 2,500 Plan: $12.30 / Individual 5,000 Plan: $7.88 / Family 5,000 Plan: $19.69 / Individual 10,000 Plan: $12.80 / Family 10,000 Plan: $32.00.
