You understand that this authority shall remain in force until you notify with Veritas Community Plus Care Plan or their plan administrator Trupoint Marketing, LLC. of its cancellation; You have given accurate information provided during this enrollment process; and acknowledge this is not insurance with the exception of the life insurance benefit. If you are not satisfied within 30 days after the effective date, you can call 336-714-7344. The Program will then be canceled and any Charges /Fees paid, as shown above, will be refunded.

This plan does not meet the minimum creditable coverage requirements under MGL c.111M and 956 CMR 5.00. This plan is not a Qualified Health Plan under the Affordable Care Act. This is not a Medicare prescription drug plan. The range of discounts will vary depending on the type of provider and service. The plan does not pay providers directly. Plan members must pay for all services but will receive a discount from participating providers. You may cancel within the first 30 days after effective date or receipt of membership materials (whichever is later) and receive a full refund.
